Are you a new patient? Here is how to make sure you are ready for you first physical therapy appointment.

• To save time, download and print the forms below to fill out before your first visit and bring the forms with you to your appointment. 
• Bring any other paperwork to your appointment that might be important for us to review and make copies of such as MD Physical Therapy referral, MRI or radiology report.
• Be sure to wear comfortable clothing that allows for good movement all around and access to the specific body parts we need to evaluate.
